In consideration of the applicant’s participation in the above activity, I waive and release all claims for damages for personal injury, death, property damage or theft that may occur as a result of engaging in that or related activities.  This discharges in advance, Beka Tennis Academy LLC, as well as their respective employees and other agents from liability and damages.  I know that this activity involves a risk of accidents and I willingly assume the risk. I also agree to accept full financial & other responsibility for any injury that may occur.

I acknowledge that Beka Tennis Academy LLC reserves the right to dismiss any student, whose conduct is detrimental to lessons, with no refund.

I understand that pictures may be taken during the lesson, and they will be the sole and exclusive property of the Beka Tennis Academy LLC and may be used in future promotional materials & websites.  This waiver, release and assumption of risk is binding on my heirs and assigns.  I have read and understand this.
